The MOR inverse agonism of naltrexone, when it is co-present with MOR agonists, may in part underlie its ability to precipitate withdrawal in opioid-dependent individuals. When binding of serotonin takes place, the protonated primary amine site forms a salt bridge with D134 residue in TM 3, as well as forming a hydrogen bond with residue S138 in TM 3. The aromatic indole ring forms a strong Van der Waals interaction with residues F223 in TM 5 and F328 in TM 6. The ring falls tight into the receptor pocket, stacked between two phenylalanines. Amine of the indole group forms a hydrogen bond with S219 residue in TM 5 and hydroxide substituent of the indole forms hydrogen bonds both with residue S131 in TM 3 and I332 in TM 6. There is also a strong Van der Waals interaction between the indole and I332 in TM 6.

=== Polonnaruwa === On 29 December 2012 a green fireball was observed in Polonnaruwa, Sri Lanka. It disintegrated into fragments that fell to the Earth near the villages of Aralaganwila and Dimbulagala and in a rice field near Dalukkane. Rock samples were submitted to the Medical Research Institute of the Ministry of Health in Colombo. The rocks were sent to the University of Cardiff in Wales for analysis, where Chandra Wickramasinghe's team analyzed them and claimed that they contained extraterrestrial diatoms. From January to March 2013, five papers were published in the fringe Journal of Cosmology outlining various results from teams in the United Kingdom, United States and Germany. However, independent experts in meteoritics stated that the object analyzed by Wickramasinghe's team was of terrestrial origin, a fulgurite created by lightning strikes on Earth. Experts in diatoms complemented the statement, saying that the organisms found in the rock represented a wide range of extant terrestrial taxa, confirming their earthly origin. Wickramasinghe and collaborators responded, using X-ray diffraction, oxygen isotope analysis, and scanning electron microscope observations, in a March 2013 paper asserting that the rocks they found were indeed meteorites, instead of being created by lightning strikes on Earth as stated by scientists from the University of Peradeniya. However, these claims were also criticised for not providing evidence that the rocks were actually meteorites.
=== Concerns about the medical evidence === Much of the medical evidence presented at trial has been criticised by experts across several specialties, including neonatology, pathology, nursing, biochemistry and forensic toxicology. Points of scrutiny have included the prosecution's assertions that the infants were clinically stable before their collapses, that alternative explanations could be excluded, and that the alleged mechanisms of harm were medically plausible. Controversy has also arisen from the fact that the original post‑mortem reports for the infants—most of whom underwent autopsy—did not identify suspicious or unnatural causes of death. In February 2025, a panel organised and instructed by Letby's defence team that was led by neonatologist Shoo Lee released findings from their own review of the deaths and collapses (see: § Expert panel investigation). The panel claimed that all of the incidents could be explained by natural causes, substandard care, or a combination of both, and claimed no evidence of deliberate harm.
